Strategies for Success: Employer Sponsored Population Health
Population Health has been an important component of stakeholder initiatives advancing innovation and transformation of healthcare to deliver improved outcomes, quality and value. Significant focus has been given on managing populations in applicable settings for health plan members, health system patients and public program beneficiaries. Attention is now being drawn to the opportunities in advancement of health management programs for employee populations, through employer sponsored population health programs.

Special considerations, issues and opportunities surround application of population health initiatives sponsored by employers. Strategies for success in this environment require identifying the components and steps in constructing a multi-year population health strategy that is balanced for impact and engagement, while structured for program measurement, monitoring and adjustment.
