Medicare Plan Patient Engagement: SCAN Health Plan Initiatives to Improve Outcomes
 
For many health plan populations, a significant patient engagement gap exists between base level outcomes involving unengaged patients, average adherence and subjective feedback; and potential outcomes involving fully engaged patients with optimal adherence and objective feedback. With Medicare plans, this gap can further widen due to the greater level of resources required to manage a senior and disabled population.

For 35 years, SCAN Health Plan has been focusing exclusively on the unique needs of seniors and others on Medicare. SCAN has developed initiatives to proactively engage Medicare Advantage members in preventive care that can improve quality outcomes. These programs include SCAN’s “Buddy” program (based on the care-navigator concept); “Health Check Records” (customized cards mailed to members to update them about preventive care opportunities); and Wellness Clinics, that are making an important and measureable difference, generating interventions that can improve the overall health of defined populations.
