Social determinants of health (SDoH) are the environmental conditions in patients’ lives – which can be social, economic, or physical – that contribute to or detract from health. Such factors can explain up to 60% of an individual’s health status.

In this webinar, Suzanne Doran and Emily Ferguson, Managing Editor and Associate Managing Editor for post-acute and case management content at MCG Health, dive into the expanding body of evidence supporting interventions to address SDoH. Suzanne Doran is the Managing Editor for the post-acute and case management content at MCG. Suzanne has been a registered nurse for over 30 years with over 17 years’ experience in case management. She has worked in a variety of care settings, including patient management in the acute care hospital setting: open heart intensive care, trauma, recovery and interventional radiology. Case management experience includes coordinating discharge from inpatient hospitalization (including acute and post-acute levels of care), complex case management, disease management and quality improvement. She is case management certified.

Emily Ferguson

Emily is the Associate Managing Editor of post-acute and case management content at MCG. She has been a nurse for 20 years, with over 10 years’ experience in case management in a variety of care settings, including patient management in the acute care hospital setting, long-term care, home health, and palliative care.
